About Personal Physician Care Community Center
Personal Physician Care Community Center welcomes adults age 50 and older. We emphasize lifelong wellness, with a focus on fun and learning. We offer many opportunities for older adults to be adventurous, learn new skills, socialize, and to meet new people. We want to “broaden our members’ horizons” through a wide variety of programs and classes. There is always something new to do and learn at Personal Physician Care Community Center.
We have worked in collaboration with many health care professionals and local health and social service organizations to launch several new evidenced-based programs including:
- Chronic Disease Self-Management
- Tobacco Cessation
- The Centers for Disease Control and Prevention (CDC) National Diabetes Prevention Program
- Food for Life (Physicians Committee for Responsible Medicine)
- Silver Sneakers Fitness
- Matter of Balance Program from the Area Agency on Aging

Philosophy and Guiding Principles
The Personal Physician Care Community Center’s philosophy, programs, and services are guided by the Six Dimensions of Wellness developed by Dr. Hettler, co-founder, of the National Wellness Institute. For further information about the Six Dimensions of Wellness click here.
Six Dimensions of Wellness
- Emotional – Degree to which one feels positive and enthusiastic about one’s self and life
- Spiritual – The development of a deep appreciation for the depth and expanse of life and natural forces that exist in the universe
- Intellectual – Expanding one’s own knowledge and skills, while discovering the potential for sharing one’s gifts with others
- Social – Emphasizes the interdependence of human beings and encourages contribution to one’s community and the environment
- Physical – Achieved through good nutrition and regular physical activity
- Occupational – Recognizes personal satisfaction and enrichment in one’s life through work

Through our mission statement, guiding principles, and the continuing support from our leadership team, and members, the Personal Physician Care Community Center’s commitment to the community and our partners, is to design and implement programs and initiatives that continually empower and challenge our members to:
- keep cognitively-active
- improve and maintain health through lifestyle changes
- expand and strengthen their social network, and
- remain engaged, empowered, self-sufficient and “in control” of their own lives
Our center serves as the “facilitator” for self-empowerment. The services and program choices are guided by both individual and cooperative decision-making, recognizing that Personal Physician Care Community Center can only be what its members want it to be.

Chronic Disease is the leading cause of death and disability in the United States – responsible for 7 of 10 deaths among Americans each year, with seniors being one of the groups most impacted by chronic illnesses
In Palm Beach County
The burden of chronic disease in Palm Beach County is overwhelming and mirrors that of most other under served populations. In Delray Beach, as in other communities, older adults need much more “hands-on” support and resources to fight the epidemic of chronic disease than is readily available in our community.

The Need for Effective Healthy Lifestyle Interventions and Education Cannot Be Overstated!

Seeing the devastating impact that Chronic Disease was having on his older adult patients Dr. David Neuman, an internist, and his son Elan Neuman, recognized that this needed to change. Their vision was to establish a Community Wellness Center for Seniors living in Delray Beach. They wanted to create a community-based program that would educate and help older adults with chronic diseases better manage their conditions, take control of their health, and maintain their independence.
Hence, in September 2014, the Personal Physician Care Community Center was established. The programs offered at the center were designed to focus on incorporating all aspects of the six dimensions of wellness – intellectual, physical, emotional, occupational, social, and spiritual – in its activities and services. In June 2015, the Personal Physician Care Community Center received formal federal designation as a 501(c)(3) nonprofit organization.
In March 2016, the center’s name was formally changed to Delray Community Wellness Center, with an “open house celebration” held in May. Delray Community Wellness Center will always remain committed to our mission and continue to offer programs and events that are in concert with the Six Dimensions of Wellness and the World Health Organization’s tenets of health.
At the end on 2018, the nonprofit entity was dissolved, and the name was changed back to Personal Physician Care Community Center to reflect its new structure, and the uniformity between the center and its parent company, Personal Physician Care.

Personal Physician Care Community Center continues to expand its programs and services to accommodate its significant increases in member participation.
